   each special occasion you listed, for the rest of your life.
  
  4) If you are paying by credit card, fill in all the info. and sign
   the authorization. (sorry we can't take checks by fax, but we will not
   hold up orders while waiting for checks to clear)
  Your account will be charged $39.00
 
   5) If paying by check, please make payable to: DJ Distributors. 
 
   6) Once you are enrolled, you'll never be charged again. This is a
   one time
